Other than information collected in relation to website usage in general, all personal information is kept within your phone and is not shared with the developers or any third parties.

The University of Sydney (University, We or Us) collects, stores and uses personal information in accordance with its privacy framework, which is accessible at: https://www.sydney.edu.au/privacy-statement.html

COLLECTION STATEMENT

The purpose of this statement is to set out the personal and health information that is collected and stored in relation to the Health Management Tool or HMT.

The HMT is provided as a website (https://healthmaintenancetool.com) and a mobile application (https://apps.apple.com/us/app/sci-health-toolkit/id1613802466) (https://play.google.com/store/apps/details?id=com.Staplesvr.HealthMaintenanceTool). These services are free to use.

No personal and health information is tracked or collected on the HMT website. Providing your personal and health information on the HMT app is voluntary, however, if you do not do so you may not be able to use the HMT app. All personal and health information entered on HMT app is kept within your phone and is not shared with the developers or any third parties.

While using the Health Management Tool website, we may collect, hold and use information about website usage to generate customisable reports to track and visualise data such as the number of users, bounce rates, average session durations, sessions by modules, page views, and downloads.

We may ask you if you can provide us with feedback from time to time through The Health Management Tool mobile application and website or by contacting us on:

sci-hmt@sydney.edu.au.

You will have absolute control over this communication and are not required to provide feedback. The mobile application does use third party services that may collect information from the app store used to identify you. Further information about this is available at:

We will use reasonable steps to ensure the security of your personal information, in line with our policies and information management practices. We do not accept any responsibility for systems that we do not operate or control, including the services that provide you with an internet connection and including for the security of those systems. Your use of those third-party services is at your own risk.

 

HOW IS YOUR PERSONAL INFORMATION USED?

For HMT Website:

  • No personal information is being collected and stored.
  • We may ask you for feedback from time to time through the HMT website or by contacting us on sci-hmt@sydney.edu.au.
  • We will obtain information about your usage of the HMT website through Cookies (Cookies).

For HMT App:

We do not collect and store your personal information on a third-party device. All personal information about you will be collected and stored on your device by you and will be used by you in the way you want to manage your health.

We will ask you to provide your personal information:

  1. To provide you with access to and use of the HMT App, including establishing and maintaining an account and accompanying security controls.
  2. We will obtain information about your usage of the HMT website through Cookies (Cookies).

Information Sharing with Third Parties

We may share the following information with third parties:

  1. SurveyMonkey
  2. MailChimp

For the purpose of providing you with access to and use of the HMT for analytics purposes (including Cookies).

Your personal information will not be shared with any other third parties or for any other purpose unless required by law.

Cookies

Most websites automatically collect personal information when you are browsing, by using mechanisms that include server logs, proxy logs, and cookies.

A cookie is a package of data that a website requests be stored temporarily on your computer (or in memory) to identify you as a visitor to that website. Cookies don’t personally identify you but are used to identify your preferences in relation to the use of the website so that it can present information that may be most relevant to you based on your previous site visits.

The Health Management Tool website and mobile application do not use cookies explicitly.  However, the website and app may use third party code and libraries that use cookies to collect information and improve their services. You have the option to accept or refuse these cookies and know when a cookie is being sent to your device. If you choose to refuse our cookies, you may not be able to use some portions of the Health Management Tool website.

The Health Management Tool website and mobile application may contain links to other sites. If you click on a third-party link, you will be directed to that site. Note that these external sites are not operated by us. Therefore, we strongly advise you to review the Privacy Policy of these websites. We have no control over and assume no responsibility for the content, privacy policies, or practices of any third-party sites or services.

Mailchimp

To provide our latest updates we may use MailChimp. MailChimp provides online platforms that can be used to create, send, and manage emails. In providing this service, MailChimp may collect personal information, including your email address and other information relating to that email address. For further information about the type of personal information MailChimp collects, please refer to their Privacy Policy.

We may use this information to manage emails relating to the latest updates to information on our website and app. MailChimp may transfer this information to third parties where required to do so by law, or where such third parties process the information on MailChimp’s behalf. MailChimp uses cookies, Web Beacons and Flash player code to collect information about when you visit the website, when you use the services, your browser type and version, your operating system, and other similar information.

MailChimp is based in the United States of America (USA) and the information generated by cookies about your use of the website (including your IP address) will be transmitted to and stored by MailChimp on servers located outside Australia.

You can opt out of our mailing list if you choose the ‘unsubscribe’ service provided by MailChimp in every email. You can also disable or refuse cookies or disable Flash player; however, you may not be able to use the services provided by MailChimp if cookies are disabled. SurveyMonkey

We will only use this third-party tool if you choose to provide feedback from our website or app and participate in a survey, and for the purpose of receiving and analysing your answers.

SurveyMonkey is based in the USA and the European Union (EU). The information generated by cookies about your use of the website (including your IP address) will be transmitted to and stored by SurveyMonkey on servers located outside Australia.

If you choose to remain subscribed to Mailchimp communications, or respond to one of our SurveyMonkey surveys you:

  • consent to your personal information being collected, used, disclosed and stored as set out in Mailchimp’s Privacy Policy, or SurveyMonkey’s Privacy Policy and agree to abide by SurveyMonkey’s Terms of Use for surveys;
  • understand and acknowledge that this service utilises a Mailchimp platform based in the USA, and a SurveyMonkey platform, which is located in the USA and the EU, and relevant legislation of those countries will apply; and
  • you understand and acknowledge that Mailchimp and SurveyMonkey are not subject to the Commonwealth Privacy Act 1988, and the department will not have an obligation to take reasonable steps to ensure that these sites do not breach the Australian Privacy Principles in relation to personal information, and that you will need to seek redress under the laws of the USA for Mailchimp, and the USA and EU for any privacy breaches by SurveyMonkey.

Both Mailchimp and SurveyMonkey hold TRUSTe’s Privacy Seal, which means their privacy policies have been reviewed by TRUSTe for compliance with their program requirements including transparency, accountability, and choice related to the collection and use of personal information. TRUSTe is an independent third party that operates a globally-recognised privacy trustmark.
