Keeping a Mood Diary will provide you and your healthcare professionals with a good understanding of your mental health and factors affecting it to be able to manage it better. It is recommended that you complete the mood diary for a minimum of 14 days to identify any pattern or trend.

In addition, your level of pain can affect your mood. If you find that your pain is interfering with your mood also keep a Pain diary. Sleep is another important factor to consider. Keeping a Pain Diary and/or Sleep Diary will help to inform your treatment. For example, if you think the worst will happen (called catastrophising) you may underestimate your ability to cope with pain, making you more anxious and depressed.

It is recommended that you complete the diary for a minimum of 14 days to identify any pattern or trend.

Keeping a Pain and Activity Diary will provide you and your healthcare professionals with a good understanding of your pain experience and what influences your pain to be able to manage it better. It is recommended that you complete the pain and activity diary up to 14 days to identify any pattern or trend.

In addition, your mood and/or sleep pattern can affect your pain. If you are experiencing substantial problems with your mood and/or sleep, keeping a Mood and/or Sleep Diary will help to inform your treatment. For example, your thoughts and emotions can either increase or decrease your pain perception, acting like a volume control dial in your brain.

Keeping a Bladder Diary will provide you and your healthcare professionals with a good understanding of your bladder function and factors affecting it to be able to manage it better.

Keeping a Fluid Diary will provide you and your healthcare professionals with a good understanding of your bladder or kidney problem and how fluid intake influences your bladder/kidney health. It also will help to monitor changes with treatment. For example, making changes to your medications or frequency of bladder emptying may result in less urine leakage.

It is recommended that you complete the diary for a minimum of 7 days to identify any pattern or trend. If completing the bladder and bowel diary, you only need to collect information on fluid intake once.

Keeping a Bowel Diary will provide you and your healthcare professionals with a good understanding of your bowel routine and factors affecting it to be able to manage it better.

Adequate fluid intake along with a good diet are essential components of any bowel program. Keeping a Fluid Diary will help you and your healthcare professionals to know whether you are drinking enough fluids. For example, if you don’t drink enough water, your intestine draws water from your food waste, making your stools very hard and difficult to pass.

It is recommended that you complete the diary for a minimum of 14 days to identify any pattern or trend. If completing the bladder and bowel diary, you only need to collect information on fluid intake once.

You can quickly and easily access all of your health information in the care plan on your device.

Quick Health Check

This Quick Health Check is an automated tool that asks a few simple questions to help you to quickly identify a problem, how severe the problem is and how much it interferes with your life. Your ratings on the severity and interference scales are combined to choose a recommended management pathway and care plan. You can access a recommended care plan for a common problem that you will find useful in self-managing your problem. The care plan can also be used as a way to promote collaborative (shared care) management of a problem with the support from your healthcare professional, including your GP. 

Please note, the Quick Health Check tool is used for one problem at a time. If you are experiencing more than one problem, this step can be repeated as many times as needed. 

You are strongly encouraged to download the SCI Health Toolkit app to use an interactive version of the Quick Health Check, where you can auto-fill information and modify or add further details as required, as well as share your care plan with your healthcare professional (via email).
